Please be sure to highlight any changes that have happened if you filled out a consent form last season.  We would like to keep our database as accurate as possible.  Thanks!

Elementary School Interscholastic Athletics Physical Examination Form
This form must be filled out by a physician. The form is good for two years unless your physician has indicated otherwise. The Physical Examination form must be up-to-date before your child will be allowed to practice. It is likely that 5th and 7th grade athletes need to have this form filled out. It is also likely that 6th and 8th grade athletes do not since the forms are kept on file for two years. Any questions, concerns contact the Athletic Director, Kevin Reitman at (414) 861-2894 orkwreitman@gmail.com.
